Need help Latinizing names, surnames, & nicknames

I'm working on an "operatic requiem" about the Jonestown tragedy, & I need some help Latinizing some of the names of the characters in the story. There are quite a handful of names that I need translated into Latin, & I've categorized them by type for your convenience:

FIRST NAMES:
Sherwin (masc.)
Juanita (fem.)

SURNAMES:
Thrash (fem.)
Amos (fem.)
Amos (masc.)
Bouquet (fem.)
Bouquet (masc.)
McGowan (fem.)
Jones (fem.)
Prokes (masc.)
Green (fem.)
Stoen (masc.)
Beam (masc.)
McElvane (masc.)
Tupper (fem.)
Dwyer (masc.)

NICKNAMES:
Don (nickname, short form of "Donaldus")
Bob (nickname, short form of "Robertus")
Jack (nickname, short form of "Jacobus")

Also, I would like to know how the Latin versions of these names, surnames, & nicknames would be declined.

I cannot help you completely for I have no knowledge regarding constructing Latinized surnames (name and surname latinisation was popular in late renaissance and baroque in Germany, France and Polish-Lithuanian Commonwealth , suffixes like “–us” or “-ius” were added e.g. Berg >> Bergius,).
Juanita (Joanna) - Ioanna
Sherwin (I am not sure, but I think I saw a reference to Ciaranus, however I underline that I'm no expert in medieval Latin, therefore I do not guarantee if it’s correct).
Donaldus, Robertus and Jacobus are correct, however I don't think that it is possible to derive direct latin nicknames from these names.
